And thank you for your interest in applying for our Case Study Program.

The “Success” Transformation Studio Case Study Program is by Invitation Only and applying does not automatically guarantee acceptance. 

So that your Application can be correctly evaluated, please complete the following survey to the best of your ability.

Only provide details where you feel comfortable sharing, however please provide enough information so that your Application can be properly assessed.

All questions require an answer, and only serious Applications will be considered.

First … Please rate yourself from 1 to 10, where 1 is Severely Displeased with your state, and 10 is your Ultimate State.

Second … Provide as many details as necessary to accurately describe your current state of affairs in each of the 12 Facets of Life mentioned at each question.

Thirdly, you will be required to follow all 4 Social Media platforms – if you haven’t done so already, simply copy and paste the links provided into a new web browser, like or follow the page/group, and then come back and tick the relevant boxes.

When you have completed the Application, Click the “Submit Application” button …

A message will be displayed which contains a link to schedule your Interview – Please Copy and Paste the link provided into a new web browser and schedule your Interview so that your application may be processed correctly.
