Case Study Program
The Acknowledgment and Release Form serves as part of these Terms & Conditions,
and both are subject to change at any time without notice.
1. The Case Study Program is offered intermittently with limited availability. As such, invitation into the Case Study Program is at the sole discretion of The “Success” Transformation Studio, does not guarantee acceptance, and no correspondence will be entered into.
2. All sessions are to be scheduled consecutively as from the 1st appointment and all are to be redeemed weekly from the date of the 1st Consultation.
3. The terms of participation in the Case Study Program are as follows …
In exchange for services provided by The “Success” Transformation Studio
3.1 You agree to provide all information requested during the application process.

3.2 Completing and submitting the application is considered acceptance of these terms.

3.3 The financial value is $1,235 for the “Head In The Game” with PSYCH-K® 5 Pack
.
3.4 The financial value is $1,670 for the “In Focus ” Business Strategy 5 Pack
.
3.5 The financial value is $2,404 for the “Back In Balance” 5-week Success Program
.
3.6 Participants in this program are required to submit the following:.
……. * Initial Intake Form outlining your current situation across several areas of your life
……. * 5 x weekly reports via the online Check In portal within 48 hours of your next session
3.7 You agree to summarise your weekly progress, your wins, your challenges, your feedback etc, with a written report or an optional short video (3-5 minutes)
3.8 Failure to provide these progress reports will result in your Case Study Program offer being terminated immediately.
3.9 At the end of your program, you agree to participate in a live interview which will be recorded in the Studio, or via Zoom regarding your experience. .
3.10 You will be given access to the final interview prior to public release for your approval.

4. Should any of these requirements not be met, you will be invoiced for services rendered at standard service prices and all legal measures will be taken to recover full payment.
5. All appointments are subject to availability, are not transferrable, and no correspondence will be entered into.
6. Rescheduling appointments are at the sole discretion of The “Success” Transformation Studio.
7. If at least 48 hours notice is given, you will reschedule your appointments using the online portal (subject to availability), providing it is still within the bounds of the time limitations of the Case Study Program Agreement.
eg. in the case of the “Head In The Game” with PSYCH-K® 5-Week Package, the “Back In Balance” 5-week Success Program, all 5 appointments must be met within the 30 days.
8. In the event that you fail to give at least 24 hours notice of not being able to attend a scheduled appointment, your continuation in the Case Study Program will be reviewed by The “Success” Transformation Studio.
9. Extenuating circumstances regarding appointment changes are taken into consideration only if contact has been initiated in writing and are arranged BEFORE the set appointment time, but this does not guarantee the adjustment.
10. You understand that The “Success” Transformation Studio is under no obligation to honour the Package or Program outside of the set time-frames and there will be no continuation if changing the appointments outside of the agreed schedule.
11. If any of these conditions are breached, continuation in the Case Study Program is not guaranteed. This decision is solely at the discretion of The “Success” Transformation Studio without obligation, and no correspondence will be entered into.
